Case Manager
 
  • 76 hour fortnight with a fortnightly PDO

About the role

Reporting to the Team Leader, you will be working within our Youth Connect program. In partnership with the Queensland Government the Youth Connect program focusses on helping young people leaving state-based care get on their feet and develop the skills and confidence to live independently.

You will be responsible for working therapeutically with young people, supporting them by providing opportunities to voice their views and wishes and then sharing these with the wider care team.

You get to watch the young people grow, mature and achieve their set goals and you can take pride in knowing you were a part of that growth.

Each day will be distinct in this varied role, however, your key everyday jobs will include:

  • Undertaking initial screening and intake processes, including risk and needs assessments;
  • Overseeing the development and ongoing review and evaluation of case plans;
  • Directly supporting and engaging with young people to provide assistance in finding education, training and employment opportunities and in developing critical life skills, social connections and self-advocacy;

 

About you

You are passionate about the welfare of children and achieving the best outcomes for them.

You are a charismatic leader who can mentor your team members to overcome hurdles and achieve shared goals.

With great communication skills, you are capable of liaising comfortably with a range of internal and external stakeholders. You have a comprehensive knowledge of legislative and service requirements and promote a culture of compliance.

To be suitable for this position, your qualifications for the role will consist of:

  • A recognised qualification (Bachelor's degree or higher) in Social/Behavioural Sciences or related discipline;
  • Solid experience in leading a multidisciplinary team in a child protection and/or housing and homelessness context;
  • Applied knowledge and understanding of child protection, trauma-informed and out-of-home care frameworks;
  • Demonstrated experience in the provision of care services to children and/or young adults in a social welfare setting;
  • 2 years case management experience (desirable).
